Metoclopramide hydrochloride (nausea and vomiting medicines)

Metoclopramide hydrochloride is a medicine which is used in nausea and vomiting, nausea and vomiting caused by cytotoxic medicines, procedures for investigating gastrointestinal disorders, relieving symptoms of gastroduodenal dysfunction and surgical procedures as premedication.

Metoclopramide hydrochloride helps to relieve stomach discomfort, nausea and vomiting. It also helps to restore normal gut movement after surgery. It works by increasing the action of the muscles in the gastrointestinal tract, so that food moves more quickly from the stomach to the lower gut. Metoclopramide hydrochloride is also used in certain gastrointestinal investigations where stimulating muscle contraction helps the investigation.


Uses
This medication is used to treat certain conditions of the stomach and intestines. Metoclopramide is used as a short-term treatment (4 to 12 weeks) for persistent heartburn when the usual medicines do not work well enough. It is used mostly for heartburn that occurs after a meal or during the daytime. Treating persistent heartburn can decrease the damage done by stomach acid to the swallowing tube (esophagus) and help healing.

Metoclopramide is also used in diabetic patients who have poor emptying of their stomachs (gastroparesis). Treating gastroparesis can decrease symptoms of nausea, vomiting, and stomach/abdominal fullness. Metoclopramide works by blocking a natural substance (dopamine). It speeds up stomach emptying and movement of the upper intestines.

What is metoclopramide?
Metoclopramide increases muscle contractions in the upper digestive tract. This speeds up the rate at which the stomach empties into the intestines.

Metoclopramide is used short-term to treat heartburn caused by gastroesophageal reflux in people who have used other medications without relief of symptoms.

Metoclopramide is also used to treat slow gastric emptying in people with diabetes (also called diabetic gastroparesis), which can cause nausea, vomiting, heartburn, loss of appetite, and a feeling of fullness after meals.

Pharmacology
Stimulates upper GI tract motility, resulting in accelerated gastric emptying and intestinal transit, and increased resting tone of lower esophageal sphincter. Exerts antiemetic properties through antagonism of central and peripheral dopamine receptors.


Pharmacokinetics
Absorption
Rapidly and well absorbed. Absolute oral bioavailability is approximately 80%; T max is 1 to 2 h (oral). Administration of the orally disintegrating tablet after a high-fat meal results in a 17% lower peak blood level and delays T max to 3 h.

Distribution
Approximately 30% is protein bound; Vd is approximately 3.5 L/kg.


Elimination
Approximately 85% of oral dose appears in urine within 72 h (50% is present as free or conjugated metoclopramide). The half-life is 5 to 6 h.

Onset
Onsets are 1 to 3 min (IV), 10 to 15 min (IM), and 30 to 60 min (oral).

Duration
Duration is 1 to 2 h.
